Summer Registration Instructions & Forms
1. Fill out registration form completely. Be sure you
have signed the form, and keep the yellow copy.
2. If you wish to use Checkfree, be sure to attach a
voided check if your choice is automatic withdrawal from
"checking."
3. Whether you choose the Checkfree or Non-Checkfree
payment plan, your first month's tuition plus the annual,
non-refundable registration fee (per person) must be paid
in full by cash or check at the time of registration. This
reserves your dancer's place in class. The Checkfree
program will not begin until your second month of class.
4. Please fill out a separate registration form for each
member of your family and total each person's fees
separately. One check to cover everyone's fees is fine.
5. Classes will be filled on a first come, first serve basis.
Popular classes fill up quickly. Incomplete registration
forms or incorrect $ amounts will delay your registration.
Please call if you have any questions.
6. To register students by mail, send your registration
form, (with a voided check, if necessary), and a check or
money order payable to Dance Attack, LLC, 120 Carroll
Street, Sunnyvale, CA 94086. You may also drop off your
form at the studio or through the mail slot.
7. No confirmation will be sent. If you do not hear from
us, you can assume you have been registered in the
class(es) you wanted. We will call you if there are any
problems with your registration.
8. Classes will be closed when full and names will be put
on a waiting list. Classes without adequate registration
may be cancelled. So please, register early!
